Case Manager

Seeking a Registered Nurse, Social Worker, or clinician with strong interpersonal skills to provide case management for Southeastern Michigan and surrounding areas. The position entails coordination of care including but not limited to catastrophic and geriatric care management. Continuum Health Management Services, LLC. provides support and education to new case managers through an extensive mentorship training program.

Our ideal candidate possesses the ability to:

  • Work independently and successfully obtain required benchmarks.
  • Think critically and problem solve by utilizing identified resources.
  • Understand and implement the nursing/case management process.
  • Communicate effectively in writing and verbally.

Our ideal candidate possesses computer and analytical skills. They are a proficient strategic planner, assertive, organized, highly motivated and flexible.

Requirements are:

  • Valid driver’s license, insurance, and reliable transportation for local travel within the State of Michigan.
  • Knowledge of insurance and billing guidelines related to auto no-fault and medicare.
  • Current valid Michigan Nursing License or LMSW preferred.
  • Minimum of 5 years of healthcare experience in case management, home care, gerontology, or rehabilitation.
  • Ability to interface with technology – proficient in Microsoft Outlook, Word and report writing.

Our expertise with auto no fault insurance allows us to provide our clients with an uncompromising level of quality health care while focusing on the need for medical coordination and cost containment. We currently serve Southeast Michigan.
